The report details two patients who developed sudden dizziness within hours of using a massage gun on the upper neck/lower scalp area. This type of vertigo, known as benign paroxysmal positional vertigo (BPPV), is triggered by head movement in a specific direction and orientation in space. Typically, BPPV occurs when calcified crystals move around in the inner ear balance system, leading to a sensation of the room spinning.

While BPPV is more commonly seen in older individuals, the two patients in the report were under the age of 50. The symptoms of BPPV are isolated to the balance system, with no other ear-related or systemic physiological issues. Treatment for BPPV involves maneuvers known as the Epley maneuvers, which are successful in alleviating the symptoms of dizziness. In the video provided, the Epley maneuver is demonstrated to show how it is performed to treat BPPV triggered by handheld massage guns.

The vertigo triggered by handheld massage guns is thought to be due to the combination of substantial force and rapid repetitive movements, which can stimulate the loosening of crystals in the balance or vestibular system. Other triggers of high-force repetitive vibrations have also been described, such as the use of electric toothbrushes and dental procedures. Dr. Ronen Nazarian, lead study author and ear specialist in Los Angeles, California, advises individuals to use handheld massage guns in the upper neck area with lower rotational settings and force levels. If any symptoms of vertigo occur, it is recommended to stop using the device and seek medical evaluation to rule out other potential causes of vertigo.

Although this entity may be rare, given the popularity of handheld massage guns, manufacturers may consider including vertigo as a potential user risk in their packaging information, especially when using the device in the upper neck area. It is important for individuals to be aware of the potential risks associated with using massage guns and to use caution when applying them to sensitive areas such as the neck. If any symptoms of vertigo or dizziness occur after using a massage gun, it is advised to seek medical attention to determine the underlying cause and appropriate treatment.
